the cornea, the clear outer window of the eye, to become progressively

of nearsightedness and astigmatism.<span style="mso-spacerun: yes;"> </span>Keratoconus

Elevated blood sugar levels, excessive thirst with an increase in urine excretion, and changes in the body's blood vessels are all characteristic of the disease. Diabetes may cause serious changes in the eyes as well. Conditions such as cataracts, glaucoma, occasional blurring and, most importantly, changes in the blood vessels at the back of the eye all may affect sight.<br />

<strong><span style="background-color: #f3f3f3; color: #4c1130;">What is Diabetic Retinopathy?</span></strong><br />

<span style="color: black;">Diabetic retinopathy is a complication of diabetes that affects the eyes. It is caused by the deterioration of the blood vessels nourishing the retina at the back of the eye. These weakened blood vessels may leak fluid or blood, develop fragile brush-like branches, and become enlarged in certain places. </span></div>
<br />
The retina is the part of the eye where light filtering through the lens is focused. The focused light or images are then carried to the brain by the optic nerve. When leaking blood or fluid damages or scars the retina, the image sent to the brain becomes blurred.<br />
<br />
The risk of developing diabetic retinopathy is high for people who have had diabetes for a long time. About 60% of those having diabetes for 15 years or more have some blood vessel damage in their eyes. When diabetes develops in childhood or teenage years, it is known as juvenile diabetes. Juvenile diabetics are particularly likely to develop diabetic retinopathy at an early age. Only a small percentage of those developing retinopathy, however, have serious problems with vision, and an even smaller percentage ever become blind. In spite of this, diabetic retinopathy is the leading cause of blindness among adults in the United States, and people with diabetes are said to be 25 times more prone to blindness than the general population. <br />

There are 2 forms of diabetic retinopathy. In one form, <strong><em>background retinopathy</em></strong>, blood vessels within the retina change. Some vessels decrease in size and others enlarge and form balloon-like sacs which obstruct the flow of blood through the vessels. These vessels leak and hemorrhage causing swelling of the retina or forming deposits called exudates.<br />
<br />
Background retinopathy is considered an early stage of diabetic retinopathy. Fortunately, sight is usually not seriously affected and in 80% of the cases, the condition does not progress. However, in some cases, the leaking fluid collects in the macula (the center portion of the retina which is responsible for central vision). Straight-ahead images, reading and close work may then become blurred, and loss of central vision can result in legal blindness. Background retinopathy is a warning sign and can progress to more serious, sight-endangering stages.<br />
<br />
The second form of diabetic retinopathy is <strong><em>proliferative retinopathy</em></strong>. This begins in the same manner as background retinopathy with the addition of new blood vessel growth on the surface of the retina or the optic nerve. These fragile new blood vessels may rupture and bleed into the vitreous, the clear gel-like substance that fills the center of the eye. If the leaking blood clouds the normally clear vitreous, light passing from the lens through the vitreous to the retina is blocked and images are distorted. Additionally, scar tissue forming from the mass may tighten and pull on the retina, tugging it away and detaching it from the back of the eye. Blood vessels may even grow on the iris and cause a form of glaucoma. Severe loss of sight and even blindness may result from these conditions.<br />
<br />
<strong><span style="background-color: #f3f3f3; color: #4c1130;">Cause and Symptoms</span></strong><br />
<br />
The cause of diabetic retinopathy is not completely understood; however, it is known that diabetes weakens small blood vessels in various areas of the body. Pregnancy and high blood pressure may worsen this condition in people with diabetes. <br />
<br />
Though gradual blurring of vision may occur, sight is usually unaffected by background retinopathy, and changes in the eye can go unnoticed unless detected by a medical eye exam. <br />
<br />
When bleeding occurs in proliferative retinopathy, the patient has hazy or complete loss of sight. Though there is no symptom of pain, this severe form of diabetic retinopathy requires immediate medical attention.<br />
<br />
<strong><span style="background-color: #f3f3f3; color: #4c1130;">Detection and Diagnosis</span></strong><br />
<br />
A comprehensive medical eye examination by an ophthalmologist is the best protection against the progression of diabetic retinopathy. (People with diabetes should be aware of the risks of developing sight disturbances and should have their eyes examined periodically because these examinations help to detect the presence of diabetes and other diseases.)<br />
<br />
To detect diabetic retinopathy, the ophthalmologist painlessly examines the interior of the eye using an instrument called an ophthalmoscope. The interior of the eye may also be photographed to provide further information.<br />
<br />
If diabetic retinopathy is noted, a second method of examination may be used by the ophthalmologist to see which blood vessels are bleeding or leaking fluid. First a fluorescent dye is injected into the arm. The dye travels through the bloodstream and passes into the blood vessels of the retina. Photographs are taken rapidly of the dye as it leaks through the retina's blood vessels. This technique, called fluorescein angiography, is sometimes used by ophthalmologists to determine if further treatment is necessary.<br />

<strong><span style="background-color: #f3f3f3; color: #4c1130;">Treatment</span></strong><br />
<br />
When diabetic retinopathy is diagnosed, the ophthalmologist considers the patient's age, history, lifestyle, and the degree of damage to the retina before deciding whether treatment or monitoring of the disease is most appropriate. In many cases treatment is not needed; in others, treatment is recommended to halt the damage and sometimes to improve sight.<br />
<br />
Probably the most significant treatment is the use of <strong><em>ophthalmic laser surgery</em></strong> to seal or <strong><em>photocoagulate</em></strong> the leaking blood vessels. This procedure focuses a powerful beam of laser light energy onto the damaged retina. Small bursts of the laser's intense heat stops the bleeding by sealing leaking vessels and forming tiny scars inside the eye. These scars reduce abnormal blood vessel growth and help bond the retina to the back of the eye. This treatment does not require an incision and may be performed in the ophthalmologist's office. If diabetic retinopathy is detected early, photocoagulation by ophthalmic laser surgery may stop continued damage. Even in advanced stages of the disease, it can reduce the chances of severe loss of vision.<br />
<br />
However, photocoagulation cannot be used in all patients. Depending on the location and extent of diabetic retinopathy, and if the vitreous is too clouded with blood, another treatment must be used. In this surgical procedure called a <strong><em>vitrectomy</em></strong>, the blood-filled vitreous is removed from the eye and replaced with a clear, artificial solution. About 70% of vitrectomy patients notice an improvement in sight. The ophthalmologist may recommend a vitrectomy soon after the vitreous becomes clouded by blood, or wait up to a year to see if the eye clears itself naturally. The timing for each patient depends on the extent of damage to the eye and to the condition of the other eye. If, however, diabetic retinopathy causes the retina to detach from the back of the eye, severe sight loss or blindness can result unless surgery is performed immediately to reattach the retina.<br />
<br />
Successful treatment of diabetic retinopathy not only depends on early detection with monitoring and treatment, but also on the patient's attitude and self-care. All medications should be taken and diet control diabetes should be followed as directed. Although physical activity presents few problems with background retinopathy, it can increase bleeding in proliferative retinopathy. Exercise for patients with proliferative retinopathy should be moderate, and straining or leaning over with the head down should be avoided.<br />

<strong><span style="background-color: #f3f3f3; color: #4c1130;">Loss of Vision Is Largely Preventable</span></strong><br />
<br />
Early detection of diabetic retinopathy is the best protection against losing sight. Even when symptoms are not noticed, people with diabetes should schedule examinations with an ophthalmologist at least once a year. Medical eye exams should be scheduled more frequently after diabetic retinopathy is diagnosed. In most cases, with careful monitoring, the ophthalmologist can begin treatment before vision is affected.<br />

Of those age 60 or over, the chances are approximately 6 in 10 that you are experiencing the symptoms of cataracts. A cataract refers to the clouding of the eye's natural lens. Because cataracts form gradually---often over years---the symptoms are often difficult to spot. Take a few moments now to see if any of these symptoms are familiar to you:<br />

<strong><u>Sensitivity to Glare</u></strong><br />
<br />
The erosion of one's ability (as compared to that at a younger age) to adjust to excessive light is an early sign of cataracts. <br />
<br />
Other indications of cataracts include difficulty when driving toward the sun, difficulty to see traffic signals against a bright sky, or a delay in return to normal vision after passing an oncoming set of bright headlights.<br />
<br />
Headaches, eye fatigue, burning and watering of the eyes when exposed to bright light---even though your visual acuity remains essentially unchanged---are also symptoms to look for.<br />
<br />
<strong><u>Difficulty Reading</u></strong><br />
<br />
Because the cloudy lens allows less light to reach the retina, vision in general and reading vision in particular, are often fuzzy and indistinct through the affected eye.<br />
<br />
You may experience the symptoms as an inability to read small print in telephone books or on medicine labels, or you may notice that reading the menu---or the bill---in dim restaurants has become a difficult chore.<br />

You may also find that reading in <u>bright</u> light is more annoying, because the glare of light reflected from the page may make your eyes burn. <br />

<strong><u>Loss of Color Perception</u></strong><br />
<br />
When the once crystal-clear natural lens becomes yellow or brown with age, it greatly decreases your ability to distinguish colors at the blue or violet end of the spectrum.<br />

After replacement with a clear artificial lens, many notice a newfound appreciation of blues---a once muddy-looking sky may now appear vibrant.<br />

<strong><u>Loss of Depth Perception</u></strong><br />

The ability to see in three dimensions requires good vision from both eyes. If you find that you have difficulty judging distances---if you undershoot the glass you were about to pick up, for example---it may signal a visual impairment in one eye.<br />

You may discover other depth-perception problems while driving; gauging the distance to the car in front of you may be harder, or you may find you run up on the curb when parking.<br />

Because less light penetrates the lens, a cataract may be most apparent at low light levels. This may become unmistakable when trying to drive at night. If you feel less sure about driving after dark, it's time to call your doctor for an appointment.<br />

As adults pass age 50, some suddenly experience the ability to read <u>without </u>their reading glasses or bifocals. While this "second sight" may seem like an improvement, it is actually a signal that the lens is turning hard and brittle; as the lens hardens, it sometimes bends light rays so that close objects can be seen without glasses.<br />

Decades ago, cataracts virtually always condemned victims to a life of near total blindness. Fortunately, today there is a relatively straight-forward procedure for the permanent replacement of the eye's natural lens with a clear implant called an "intraocular" lens. <br />
<br />
Intraocular lenses are about the size of Lincoln's head on a penny and weigh about the same as a kernel of corn. They are polished to the precise curvature needed to bend light rays into focus on the retinal at the back of your eye.<br />
<br />
The intraocular lens is made of an inert plastic and can provide good vision for the rest of your life. Many lenses even screen out potentially harmful ultraviolet rays from the sun. Some lens implants are foldable- this allow the implant to be inserted through a smaller incision.<br />
<br />
<strong><u>What to Do</u></strong><br />

<strong><u>Cataract Surgery</u></strong><br />

For most cataract patients, the intraocular lens implant literally replaces the natural lens in its position behind the pupil opening of the eye. <br />

After anesthesia has taken effect, a small incision is made to first remove the hard center (nucleus) of the lens, then the remaining cloudy, milky contents. Under the shell (skin) of the lens which remains is a space for the eye surgeon to implant an artificial lens specially prescribed for your eye. A surgical microscope aids the surgeon in removing the cataract and in placing an artificial lens within the eye. <br />
<br />
Although surgical time varies, the operation usually takes from 20 to 30 minutes. Most patients are at the surgical center for at least 2 hours on the day of surgery.<br />

fairly quick procedure.<span style="mso-spacerun: yes;"> </span>Most patients
are in our surgery suite for 15 minutes total for a procedure on both
eyes.<span style="mso-spacerun: yes;"> </span>Improvement in vision is often
immediate; however, the patient’s vision will be hazy for the first 24 hours.
After surgery, someone must drive the patient home.<span style="mso-spacerun: yes;"> </span>The patient is given comfort goggles to sleep
in to protect their eyes (these goggles are worn at night when the patient
sleeps for 1 week). The patient must go to sleep or keep their eyes closed for
at least 5 hours or longer following surgery.<span style="mso-spacerun: yes;">
</span>When the patient wakes up, they are advised not to put any strain on
their eyes.<span style="mso-spacerun: yes;"> </span>Excessive television, smart
phone usage, computer activities, and reading fine print should be avoided if possible.<span style="mso-spacerun: yes;"> </span>These activities can be performed as soon as
the next day after surgery; however, the patient’s eyes may tire faster than
normal, so it is important for them to take breaks when needed.<span style="mso-spacerun: yes;"> </span>Many patients will experience redness of the
eyes along with mild burning, stinging or a scratchy sensation for a few days
after the LASIK procedure. Some patients have light sensitivity and will
require dark sunglasses for a few days or weeks following LASIK surgery.<span style="mso-spacerun: yes;"> </span>It is normal for vision to fluctuate after
surgery. The period of fluctuation varies and is usually due to dryness of the
eyes.<span style="mso-spacerun: yes;"> </span>Some patients will have more dry
eye than others and may have to use more artificial tears.<span style="mso-spacerun: yes;"> </span>Artificial tears are usually required for a
few months after surgery and how often they are used varies from patient to
patient.<span style="mso-spacerun: yes;"> </span>As far as driving is concerned,
most LASIK patients see well enough to drive and some can even return to work

patient works in a dusty environment or outdoors, we may recommend that they

surgery, patients are placed on a strict eye drop schedule. We prescribe an
antibiotic drop and a steroid drop that are both used 4 times per day for 1 to

also instructed to use over-the-counter artificial tears 4 to 6 times per day
or more.<span style="mso-spacerun: yes;"> </span>Patients cannot rub or bump
their eyes for several weeks following surgery.<span style="mso-spacerun: yes;">
</span>Women cannot wear eye make-up for 7 to 10 days following surgery.<span style="mso-spacerun: yes;"> </span>Patients are instructed to avoid swimming,
sauna, hot tubs, water park rides, etc., for 1 month.<span style="mso-spacerun: yes;"> </span>The patient should not perform any yardwork for
2 weeks. Exposure to chemicals or smoke getting in the eyes should be avoided
for at least 2 weeks.<span style="mso-spacerun: yes;"> </span>Light exercise can

The PRK procedure is similar to that of LASIK in the way that the laser is applied to the eye. The main difference is that there is no flap creation. Instead, the laser is used to produce your optical correction by reshaping the outermost surface of the cornea, rather than the tissue beneath a flap, as in LASIK. This requires the removal of a thin layer of corneal epithelium, <u>which may produce varying degrees of temporary discomfort for up to a few days after your treatment</u>. In general, PRK provides a slower visual recovery than LASIK. <u>It typically takes 1-2 weeks or longer for patients to achieve their best vision.</u><br />

Dr. Bogart prescribes additional medications and places a thin, soft bandage contact lens over the eye to protect the eye and minimize discomfort for the first few days after treatment as the cornea heals. Surface Ablation is recommended for those patients:<br />

<li>whose prescription is too high to have LASIK safely</li>
<li>whose corneas are too thin to have LASIK safely</li>
<li>whose corneas display evidence of scarring from infection or trauma</li>
<li>who have had a previous LASIK procedure over 5 years ago</li>
<li>who live an active lifestyle, military personnel, police officers, firefighters, athletes, kick-boxers, martial artists, or those who are in or around dust environments, chemicals, or smoke on a regular basis.</li>
<li>whose primary goal is SAFETY, PRK is the safest procedure for anyone considering refractive surgery.</li>

ambulatory procedure; you walk into the surgery center, have surgery, and walk

</span>Your eyes are first anesthetized with special drops.<span style="mso-spacerun: yes;"> </span>Dr. Bogart will have you lie down, to make
sure your eye is positioned directly under the laser.<span style="mso-spacerun: yes;"> </span>(One eye is operated on at a time.)<span style="mso-spacerun: yes;"> </span>A kind of retainer is placed over your eye to
keep your eyelids open.<span style="mso-spacerun: yes;"> </span>Normally, this
is not uncomfortable.<span style="mso-spacerun: yes;"> </span>It has a suction
ring that keeps your eye pressurized and immobile.<span style="mso-spacerun: yes;"> </span>The pressure feels awkward, but does not
hurt.<o:p></o:p></span></div>

by gently removing the eye’s protective first layer of cells, or epithelium
(this will naturally regenerate itself in a few days), to reveal the next layer
of corneal tissue known as Bowman’s layer.<span style="mso-spacerun: yes;">
</span>He will then smooth the area and proceed with applying
computer-controlled pulses of cool laser light to precisely and delicately
reshape the curvature of the eye.<span style="mso-spacerun: yes;"> </span>Deeper
cell layers remain virtually untouched.<span style="mso-spacerun: yes;"> </span><o:p></o:p></span></div>

treatment itself is usually completed in less than five minutes and is

as a human hair is typically removed, the cornea maintains its original
strength.<o:p></o:p></span></div>

Ablation patients are placed on antibiotic drops, along with anti-inflammatory
agents to promote comfort and reduce swelling.<span style="mso-spacerun: yes;">
</span>Patients are also fitted with a bandage contact lens to improve their
comfort while their eye is healing.<span style="mso-spacerun: yes;"> </span>Once
the epithelium has healed, usually on the third or fourth post-operative day,
Dr. Bogart will remove the contact lens bandage.<span style="mso-spacerun: yes;"> </span>Sometimes, daily eye examinations are
required.<o:p></o:p></span></div>

procedure, Dr. Bogart will have you rest for a bit, then you can go home

should try to sleep for at least the first 4 hours, or longer.<span style="mso-spacerun: yes;"> </span>You may be able to go to work the next day,
but many doctors advise a couple of days rest instead.<span style="mso-spacerun: yes;"> </span>They also recommend no strenuous exercise for
up to a week, since this can traumatize the eye and affect healing.<span style="mso-spacerun: yes;"> </span></span></div>
